This video is floating around in several places. I try and point out clues and things that could have prevented this. In the second video I show what happens when a horse bucks and falls and no humans are involved.

Could this have been prevented? Whose fault was it? Everyone but the horse.
I was asked what could she have done to prevent this, here is a short list:
She could have rode the horse before, got on and got off, got on horse without dress and been led around so horse would know what to expect, got on horse on left side so it would be more normal, have someone competent holding and leading the horse who could actually control the horse, pick up the reins when she got on so she could do a one rein stop if the horse tried to run, put another horse in front of horse so both horses find comfort and a horse won’t run over another horse like it will a dummy, have someone next to bride to grab or pull her off if the horse spooked or ran off, backed the horse up at the first sign of him wanting to move forward, not have a bunch of people around the horse causing more pressure, face the horse towards the water not too many horses will want to run into the ocean, that is just a few that pops into mind, but I am sure that are many more things.

The problem is that if the "professional" person DOESN'T try to help, then that person is probably going to be sued. People would say, "Why didn't you try to help?" Of course, in a case like this, the trainer/ horse-owner is probably going to be sued anyway. I don't know if you'd have to be supremely self-confident or be completely crazy to let strangers in wedding dresses sit on your horse for photos, but when you're dealing with the public (and in a public area, no less), SO much can go wrong. I don't care how well-trained, the best horse in the world can just decide he's done, or he can be stung by a wasp, or some car nearby can backfire and the horse go nuts.

Most probably the wedding horse was never sacked out for that type of shooting and the all white dress flowing over his butt really got to him along with the crowd ,lights, etc. Probably first time doing it.
